With help from Peter King and Ken from S&K Electric we determined that the disconnect was turned off for the H-2 PSL make up air fan. I turned the disconnect on and the fan is working properly now.

The fan disconnect was not labeled, however, Peter said he would print a label and install it on the disconnect.
