Today, we removed the actuator covers from GV9 and GV10 so as to verify the presence of a spindle nut (retaining nut?), anti-rotation washer (spindle nut lock washer?) and to record the ball/detent torque pre-load as indicated on the graduated scale. In both cases, the nuts and washers were present but neither appeared to have a tab if the washer bent so as to engage with the key slot on the shaft or nut.  This has been the case for all valves inspected up to this point.  My understanding is that this nut and washer don't appear in the GNB drawings and may have been added during installation(?).  Without documentation, we aren't 100% sure as to what was intended but, typically with the arrangement, one tab of the washer would be bent to engage with shaft's keyed slot and a different tab would be bent to engage with the nut so as to prevent the nut from rotating independent of the shaft. 

I didn't have my camera with me at the time but both GV9 and GV10's "pointer" capscrews were aligned with the "4" position.  Three thread crests were showing above the nut.
