Looks like the lock loss was caused by an ADC error on h1iopseib3. Erik is currently working on the issue.

Failure happend at 13:16:22 UTC.
The second ADC showed timeout. On reboot of the front end, the Linux kernel crashed.
Richard cycled power on the IO Chassis and we rebooted the front end again. This time it did not crash, but could not see the second DAC.

Richard is going to re-seat the second DAC.
Richard found that the LEDs on the second DAC did not come up as they did with the first DAC. The second DAC again did not appear on the PCI bus.
Richard is going to replace it.
We're back up and running with the new DAC.
